Consent form

This consent form must be completed before anyone can take part in activities and experiences.
Fat Squirrel Outdoor Co ltd aims to provide a safe and enjoyable experience for everyone taking part.
You are agreeing to follow the instruction of Fat Squirrel Outdoor staff to ensure everyone stays safe.
You are agreeing everyone taking part is fit and healthy enough to do so.
You will tell staff if anyone in your group needs additional support.
You will tell staff if anyone has allergies.
Fat Squirrel Outdoor Co Ltd reserves the right to stop activities if anyone's safety is being put at risk.
A responsible adult (over 18) must be present during the activities.
No alcohol can be consumed before or during axe throwing, fire lighting or other adventurous activities.
Fat Squirrel Outdoor cannot take responsibility for any lost or damaged clothing and / or personal items during the activity.
Under 18s remain the responsibility of the adult accompanying who must ensure they are kept safe around the fires, have sufficient water, food, clothing, sun lotion and medication (where appropriate) for the duration of the activity.
Fat Squirrel Outdoor will keep a record of this form for health and safety reasons.
